I have recently encountered this issue for the first time myself. I ended
up:
1) Creating the icon image in Fireworks (but you can use any graphics
progam, including the crappy but free MS Paint) and exporting it as a bitmap
(.BMP) file.
2) Downloading an icon-maker program (the one I used was Icon Forge
[http://www.cursorarts.com/ca_if.html], but there are many out there in
shareware land), and importing the bitmap (.BMP) graphic into it.
One thing I learned: Renaming a bitmap file with an .ICO extension DOES NOT
work!! You really need to bring it into a progam for converting files into
icons.

I'd like to apply a custom icon to a little utility I've created, but I need
to create a graphic file with an .ICO extension (an icon file).
Does anyone know how to do that? I have Photoshop, which I've used to create
the graphic. But it doesn't allow me to just save it as an .ICO.
