Although I am a bit confused as to the question, what about using captions
in Word?
Here is a clip of the Help file on adding captions to existing items in Word
97:

Add a caption to an existing table, figure, equation, or other item

1 Select the item you want to add a caption to.
2 On the Insert menu, click Caption.
3 Select the options you want.

Note Captions are inserted as fields. If you see an entry that looks
something like {SEQ Table \* ARABIC} instead of text, the field codes are
being displayed instead of field results. To see the field results,
right-click the field code, and then click Toggle Field Codes on the
shortcut menu

It's not a button, but it does the trick for adding text to pictures, etc.
