There is no rule in modern usage. Examples:
Janice Payne Gibson (uses maiden name as a middle name)
Janice Payne-Gibson (clings to her alphabetical position for reasons that
may have to do with leaving her banking, school, club, etc. records
undisturbed)
Janice Gibson-Payne (convention of using her husband's surname first; often
bestowed upon children in this form)
Mario Diaz-Cortes, Mario Diaz Cortes, Mario Diaz C.(Hispanic custom of using
both the patronymic and then matronymic.)
***Just ask the person the preferred format of her name.*** --Yours, Diane
Plassey Gutierrez, (esposa de Edgar Gutierrez Diaz.)
