Information Mapping and PDF

Here is a question for all you Information Mappers out there who also produce
their documents in a PDF format.

Question: Do you still maintain all levels of Map Headings at the top of the
page? Or do you now let them fall wherever they happen within the body of the
page?

We are trying to determine whether the occasional dead white space at the bottom
of a page is detrimental to the reader when viewing/reading it in PDF.

Please let me know your reasoning behind which choice you made. And if you have
made any other alterations to make Information Mapping look better in PDF.
