I like Dave's suggestions and reasons for them. I add one additional
feature if I have the choice: top level section headings under the
chapter head in the top outside corner of the header.

This helps the page-flippers find information. As (admittedly flimsy)
proof, one of my previous clients did a usability study that found
that a large percentage of users are page-flippers. As completely
subjective proof, I am an inveterate page-flipper.
