Easy using Windows 95.
Create the image as a bitmap (BMP) and save it on a Windows 95 machine.
Now go into Explorer and rename it with an ICO extension.
Windows 95 does the rest, apparently - including the scaling if it's not
exactly the right size.
The above may also work in NT, but I've not tried it.
