Request for documentation form

Helen Obaze wondered if anyone else has a "request for
documentation" form that they use to formalize such
requests. We have one for graphics (since all docs are part
of the annual study plans and thus require no additional
<ahem> documentation). We based it around the notion of
"the 5 w's": who, what, where, when, and why? ("How" is
another option, but I consider this to be part of the
previous 5 categories). Answering these questions tells you
most of what you need to know to produce the docs. If you
want more details on the 5 w's, check out my article on
this subject in Technical Communication (vol. 43, issue 2).
