Thanks to all who helped with this problem: "Can't cut and paste
fields-generated table of contents and index."
The solution was so simple: "Convert the contents of the TOC and INDEX fields to text
before copying it.
In order to do this, mark the table of contents or index and press CTRL +
SHIFT + F9 ."
