Thanks, Wayne!  I checked my copy at lunch. Mine is a little newer,
copyright 1993. Author is Peter R. Scholtes. Phone numbers for Joiner

When I originally posted about this, I had forgotten that the book was
geared toward quality improvement teams. But even so, I think the book
contains a lot of good info about team management and team dynamics that
would apply to any sort of team.
