On behalf of our group, let me join the majority re: procedures.
We use the style where the numbers reflect a significant action the user is to perform. If any explanatory or contextual info is needed, we follow with a "List Number Skip" style which indents comments below the relevant step (similar to the style presented by Ann Amsler).
If some relatively trivial step (like Click Continue) follows a significant one, then that might be incorporated into the first step with an "and then".
Within the group, we have a bit of a dilemna about whether screen shots of every single step need to be provided. Some of us feel we can give our readership some credit, others prefer the exhaustive (exhausting?) approach.
