Sometimes, by strict decree. Two companies that I worked for kept writers,
editors, and illustrators in three distinct units. Each had their own area
of assigned tasks, and did not cross over into the others. Illustrators only
did artwork (and in fact a union agreement prevented writers or editors from
making -any- changes to the art boards). Editors -never- started from a blank
sheet of paper, at one company there were folks who had been in that position
for many years and never written a procedure. Writers did the writing, but
were not permitted to do an "official" edit (i.e. sign off the paperwork
indicating the edit phase was complete).
Needless to say, this system has its drawbacks, but on a day-to-day basis
it actually functioned very well.
