Change pages are ALMOST NEVER inserted. Issuing quarterly perfect-bound
manuals doesn't really cost that much more than issuing annual 3-ring-bound
versions and issuing frequent change pages. And it works out a lot better
and is usually easier from a maintenance standpoint.
