I always say:

Select the File/Open menu item.

I have to say that an instruction like

- To open a file, select File / Open from the main menu.

is easier to read and follow than

- To open a file, select Open from the File menu.

I was going through the tutorial in the FrameMaker manual (looking at our
demo version - YAY!), and the whole manual is written with the menu name
last. This is really annoying, because sometimes the thing you're looking
for is nested several layers deep, so you'd get to a sentence that says

Select Headers/Footers --> Insert Page # from the Format menu.

I have to scan to the end of the sentence to find out which item I should
be clicking *first*. I much prefer to list the items in the order the
user will be clicking -- and as a user, I much prefer to read
instructions listed in this way, too.
