We use a variation of entity relationship diagrams based on the methods
Edward Yourdon describes in his book, _Modern_Structured_Analysis_.

They're not the most beautiful things in the world to look at, but they
do represent the database in a format that is familiar to the techie types
who want this sort of information. It also helped us to establish a standard
format for the diagrams, and (so far) it's been able to handle everything
that development has thrown at us.
