But heaven forbid that someone might want to print it, right? Do TOCs and Indexes look old-fashioned too? If you've heard that it's common in some realms to print PDFs, why would you declare that those PDFs should be less useful to those customers?

Looks old-fashioned? Bah humbug. It also looks old-fashioned for there to be enough contrast in a user interface to actually read the text. It also looks old-fashioned in a user interface for the scroll bars to always be visible and wide enough to be noticed.
