Yeah. I have trouble with this, too. We use workflows for releasing
published vs draft information, but the system is not used for tracking
reviews and sign off.

That has to be done manually, where you reviewers test the doc against a
definition of done. Then, any future changes have to be weighed as a fix to
the expression or a need for additional technical review.

One and done is not possible (or desirable) in Agile. But you have to pick
your battles.
