I recently had an interesting back and forth with a customer liaison that is apparently new to the publication end of their business. I had sent him proofs of the book cover and book pages (two separate files).

In his response, he kept referring to the two covers, and why are there two covers. Please get rid of the second cover.

It took me a while to realize that he was thinking that the title page (1st page) was another cover of some sort. Granted, the title page has a similar look to the cover. And yes, if you didn't know anything about the layout of most books (in particular, the layout of HIS book), it could be mistaken for a cover. I didn't fault him too much, and explained what was going on. His answer was, "Fine, if it has to be there, then leave it." His annoyance was pretty clear.

Last Fri I sent out a pdf to a larger than normal team with a 1500+ document for review prior to sending out for hard copy printing. I get a comment back that said, in effect, what was I doing...the content was all over the place and i better shape up or ship out.

An hour later, we figured out that what he saw (and he confirmed) was the alternating inside margin width from looking at the wider inside margin necessary for binding.
