I never survey to check my performance. Not that I am so great, but it is a
waste of time, since most of the engineers and programmers will respond with
even less enthusiasm to a survey than to the requests for information or
requests for review that I send out now.

I ask during the writing and review process "what else do you need?" and
then I provide it. If they don't answer, I don't provide it.

How do you survey your SME community to gather feedback on writer's
performance? Do you send a formal survey, use email, forms, etc.?

Do you use an internal email format; or, something like Survey Monkey to
gather feedback? What kinds of questions do you ask?

Any samples would be most appreciated.
