One of my reviewers has requested the ability to be able to navigate
through the changes in a pdf document using bookmarks or something similar.
Documents already include change bars to highlight changes, so they can
scroll through and pick them out visually however the ideal solution would
be for each change bar to have a corresponding bookmark in the final pdf.

I could do some post-processing and add empty 'comments' so that they could
use the review/comment functionality, however that is not ideal.

Any ideas?
