It sounds like you may have misunderstood the double context of "documentation." The work is documentation, but the communications should be documented as well, since verbal communication has zero standing in a dispute. Kari's boss is denying her access to SMEs but expecting Kari to document the "prerequisites" that the SMEs are doing. If this direction is verbal only, then Kari's boss can say she was clear in her expectations. If Kari documents the communication, then Kari has standing in a dispute.

If her boss is gunning for her, chances are not in her
favor that some higher-up will want to read her dutiful documentation of
what her boss told her, and what she did, etc. etc.

You are misguided.

It's far more likely, I
think, that the upper boss will simply rely on what Kari's boss says. In
employee-said, manager-said situations, directors tend to side with
managers.

That is why Kari needs to support all relevant communication with documentation, usually, emails.
