I think the thing that probably saved MS on this point was the number or times a user clicks on "Start" during a session.  By the time you want to shut down the system, you've seen "turn off computer" go by so many times that it has burned its location into your brain.

I never used the drag-to-can method of ejecting discs on my Mac Plus, but stuck to the "eject disc" menu command.  There was always a nagging voice in the back of my head telling me that one day the system was going to take that trash can analogy seriously.
