If your primary aim is to improve the quality system, then assigning
blame is worse than useless -- it's a distraction. What matters is not
"whodunit" but rather _what was done_ (or not done).
When you focus on the people responsible for the error, everyone else
thinks that they're OK, they did everything right, they'd *never* do
something as dumb as that. But the quality system should assume that
everyone's capable of similar errors.
When you focus on the _error that was committed_, you can determine the
proper training and auditing to prevent its recurrence. You can also
predict future, similar errors more accurately and take preventive steps
against them.
