Doc version control - in-house

QUESTION: In your several-hundred-employee (or more) 
company, that does product development (specifications, 
engineering, code, testing, lifecycle, etc.), what system 
do you use to control your working documents, like 
MRDs, SOWs, Test Plans, Test Reports, Work Instructions, 
etc.?? 

What automatic system of control do you have to 
enforce review-and-approval procedures, and version 
control of all those in-house docs?  Not to be 
confused with customer facing docs, which are a 
different story.

Bonus if it's a function of, or an add-on-to, or a 
plug-in for SharePoint, or MKS source control, 
or Oracle ERP (with or without the Agile add-on), 
since we already have those.
