Just another theory:

Taking into account that the traditional print process always creates
products in 4-page units, and the originating size of most printer's
paper is the broadsheet (approx 24x36), chances are that J&J has a set
process for printing and slicing all manuals to minimize the time
spent swapping plates between jobs. They probably standardized on this
size a long time ago and found it works for them.
