What you've done is a work around. Glad it worked for you. However, in
reference to your original example


1.2 - DOCUMENT EVOLUTIONS 4
RELATED DOCUMENTS 4
1.3 - 4
TERMINOLOGY 5
1.4 - 5

It should, of course, look like this:
1.2 - DOCUMENT EVOLUTIONS 4
1.3 - RELATED DOCUMENTS 4
1.4 - TERMINOLOGY 5

Here's something that might help in future projects, or if you still have
the original document should the problem occur.

Why is this happening?

I am making a few assumptions in my diagnosis. Assuming all headings are
formatted with styles Heading 1, 2, 3, etc

In the actual content, not the TOC, i.e 'RELATED DOCUMENTS' on page 4, was
probably not formatted to the style 'Heading 1'.

Essentially, in the styles pane, make sure that the line for RELATED
DOCUMENTS has been formatted to a style Heading 1 or make sure that it is
the same style as Document Evolutions.

Also it may help, for your troubleshooting scenario, to turn on Paragraph
Formatting marks/symbols temporarily...These are all the hidden paragraph
and line break markers that people tend to hide.

For the title 'Terminology' it looks like there are empty or blank lines.
Blanks could have have been applied with a style heading. Delete them.

To find the styles applied to a para or line, show the styles pane:
Go to Tools > Options > View tab > style area width > and set that value to
1". This simply shows styles that have been applied line by line. (this is a
super tip)
This is happening because some empty lines have been formatted as Heading
1s. Look for those empty lines and delete them, then regenerate your TOC.


if you can avoid, make the changes to the content/headings not the
autogenerated TOC table as these can be overwritten easily.

Hi again everyone,

Thank you for offering your help so fast.

Unfortunately, my document contains confidential information so I cannot
send it to anyone for further investigation.

I've found a by-pass solution to my issue: I've deleted all troublesome
headings, typed them again, left one empty paragraph before and one after,
applied the appropriate heading style. And now the TOC generates fine. This
works though because my document is not long and only a few headings were
screwed up.

Again I really appreciate your help. Thanks!
