At my last job, we had a standard set of deliverables, so mostly we
told engineering and marketing what we'd be delivering. If the
standard set was inadequate for a particular product, that usually
came up and was dealt with in interdepartmental planning meetings.

For efficiency, the writers mostly worked directly with the product
managers. We ran into some problems where the PMs would give the
writers projects they shouldn't have been working on. To avoid that, I
set a policy that the writers were to refer all requests for
additional deliverables to the documentation manager for refusal.

We didn't have any forms, but I did document the informal process with
wiki pages that showed which docs were in the pipeline, who was
working on them, and major milestones.
