Where I used to work, documentation was tracked in the bug tracking software. What helped was that every issue entered into the system was reviewed at a regular meeting. Work was not done on a bug until after the reviewing team had assigned the person responsible. Ambiguities could be rewritten or assigned back to the original submitter. Sometimes the administrator would take on the task of splitting multiple issues out into one problem per entry.
These reviews helped to smooth the process and also served to educate users in what was and what was not a properly written bug report.
