We use Madcap Flare. But its because we develop software and use its
single-sourcing capabilities to generate HTML Help for product integration
and regenerate PDF/Word versions for different purposes. 

If you're just looking at small how-to docs two,three times a year, you
might just use Word and publish it to Sharepoint. But if you're looking for
decent documentation support for your products, consider Flare, Lingo. Its
best for small to medium sized shops, although newer growing IT companies
are using it as well. There's a veteran bunch of long time tech comms people
working there so its quite a product with some heavy community support and
marketing.

XMetal for xml files tied to Documentum for content management.

Why? Because we AREN'T a small IT department.
