We use a template/table that includes a value for each of the
error/correction/improvement needed in the document. Values can be
anything you want, such as (M)ajor problem and must be corrected for
equipment to run. (A)ffects documentation and/or procedures but not
mandatory to fix if not enough time.....etc.
With a value system, you have at least documented the errors. It is then
up to the management chain to decide if it can/has to be fixed.
