In this case, it appears to be a glitch in the server-side
page-processing code, which generates a confirmation email each time
the page is refreshed. The behavior isn't necessarily due to tabbed
browsing; if one were to use a non-tabbed browser, but one that
"remembered" the last page open in it, and refreshed it on startup -
that would yield similar results. The same would most likely occur if
the user had to hit the Refresh button a few times to reload the
summary page, in a non-tabbed browser like IE6.

The reason why it's linked with tabs in this case is simply that with
a plethora of tabs, it's easy to overlook one or two that should have
been closed. I've had similar moments when I shut down without closing
a couple of YouTube or other video tabs; on next restart, I wondered
why on earth my Gmail was so slow... till I found out what other tabs
I had left open.
