MS Word has a simple invoice template. I put my weekly hours on one line and then total them for the month at the bottom.

July 1-4 27 hours X $xx = $xx (I did 9 hr/day cos 0 for July 4)
July 5-11 40 hours X $xx = $xx (regular 8 hr/day for 5 days)
etc.
