Fred Ridder pointed out that there was some additional useful information
that I really should include in my question, so here it is (and I hope I
got it all, this time).

1. I have no table of contents, so I cannot speak as to whether
Word-created TOC hyperlinks would work or not.

2. I'm using the Convert to Acrobat PDF command installed by Acrobat.

3. The conversion settings have "add links" and "add bookmarks"
enabled.

4. When converted, the bookmarks pane shows the links based on Word's
heading (and other selected) styles. Those work perfectly.

5. The hyperlinks I created in Word using Insert Bookmark and
Hyperlink (Ctrl+K) work as hyperlinks IN WORD but do not convert to
useable hyperlinks in the PDF. They are not supposed to appear in the
bookmarks pane, and luckily neither Word nor Adobe has seen fit to "help"
me by putting them there.
