RD Fields (MS Word 2003)

I am working with MS Word 2003

I have 5 separate files.
I succeeded to generate a TOC with RD fields for 4 of the 5 docs.

Doc # 4 is included in the TOC but does not display the title of the
document (i.e. Part 4)
Part 5 (title) does not display anything, including title.

Also, how can I include heading 7 and 8 (Appendix) in the TOC?
