I see a few tools on the web that can document the structure
and render hypertext reports for you. You may find something
for flagging changes to database structure, but you also may
end up having to create a custom report to automate that.

Web search terms: SQL Server self documenting

A lot of technology writers seem to be excited about putting
database object descriptions in extended attributes.
Whatever the value of these descriptions in your team's
databases, I do at least find tools for getting nice
hypertext reports on the structure. That's a pretty good
starting point for collecting the descriptions of the data,
no?
