This might be on the site Tammy suggested, but check
your "Normal" template to be sure there are no styles in
it with the same names as the ones in your document
template. When I create custom styles, in addition to
all the things that you listed I give them names like
"companyname_heading1" that I know will not echo
with existing Word styles, but if one of those new styles
gets copied into Normal and then altered, when a doc
is opened conflict between differently formatted styles
with the same name in the doc and Normal usually end
up ruining my day.
