What you say is true... I'm taking it from a standpoint from my early days as a typesetter, before the onset of desktop publishing -- set up the document (or "file" literally from the typesetting days) so that it will be clear what was done if anyone else goes in to edit it.
From the standpoint of starting out, it's been my experience that it's easier to follow if specific styles are applied rather than TC field codes, but I suppose to some degree it is just the preference of the documentation manager. From the standpoint of building indices, I've always favored concordance files over hard-coding index entries, but again, that's just a preference.

This is true in general, but the situation is not as absolute
as you make it out.

One exception is MS Word, where you can insert TC field
codes to define arbitrary TOC entries that are completely
independent of paragraph or character styles.

Another exception is structured authoring tools, where
the markup you use to identify elementsÂthat will be
compiled in the TOC is semantic rather than style-based.

Regardless of what tool, the first step is to ensure that you're using some form of styles. Whether you're using Word, FrameMaker, PageMaker, InDesign, or anything else, there needs to be a continuity so that predefined styles -- not just type specs, but styles -- are applied to the different headings. All the TOC-building engines work off this.
