When I first started 20+ years ago my first job was documenting from the
source code with help from specs/design docs. Most important tools were
multi-coloured pens that were used to keep track of BEGIN-END, IF-FI etc
:-)

You soon became very good at recognising the programmers by name - those
who coded 'neatly' with useful comments etc and those whose names filled
you with dread, no structure, comments...

Not done it in a while but then I've been doing end-user docs recently
bit for people documenting the APIs etc I'd say it wouldn't be that
uncommon.
