Alright. I screwed that up. Everything I said about TOC handling is true,
but the "Index 1" and "Index 2" stuff picked up entries in the index field
and not the document, so my hyperlinked index was wrong.

Maybe someone else can do a better job. Also, TOC sorts by page number and
not alphabetically, like INDEX, so that is an issue too.

INDEX doesn't use the hyperlink switch and TOC will pick up styles, but sort
by page number.
