This approach to document control seems perfectly reasonable and is
what has been the default in most of the companies I have experience
with, except perhaps the part about the PMO having to approve all
meetings between writers and SMEs, Usually, before a revision can
be made to a document that has been released or submitted for
release, a change request must be submitted and whatever person,
dept or change management group owns the responsibility for
approving must sign off before any work on a document revision
can begin.
