For more information and a basic sample of a VS Add-in, see
http://msdn.microsoft.com/msdnmag/issues/02/02/VSIDE/. The process is a
lot more streamlined for .Net than when I was working with 6.0, so it
should be a good intro if you are using .Net. The sample uses the
command line to launch IDE command but you'll get better mileage from
attaching the command to a menu or tool bar item.

Oh great, now I'll never get over the nostalgia of API documentation.
Aaah...the scent of code, the beauty of libraries, the mellow sounds of
processors compiling, the sweet taste of victory over debugging errors.

Sigh...now back to end-user documentation.
