Several times I have asked "Would you consider signing a brief letter of
recommendation?" If the response is positive, I produce an already prepared
letter that outlines the position and products in positive terms and ask
him/her to look it over with the caveat "Feel free to mark it up or rewrite
it." It usually contains their contact info and states their desire to be
contacted as a reference. They have always signed it as presented.

ps - don't overlook an opportunity whenever a manager or senior staff leaves
the location or company. That is often a chance to get a good reference to
bank against the end of project or next downsizing.
