One possibility: The QA plan describes all of the phases of the
software's design and development. One of these phases is a test
protocol, which describes how the software will be verified and
validated. Other phases include the actual verification and validation,
which are supposed to follow the protocol.
In this system, the QA plan and protocol are each unique for the
specific software under development, but each one follows a basic
structure used by the company for anything that's developed.
Some possible QA protocol elements:

* Detailed test procedures

* Personnel skill set required
* Data preparation, including randomization
* Result collection, dating, and tabulation

* Result inclusion/exclusion criteria
