My personal favorite has to do with picking the wrong suggestion from a spell checker.
In the early 90s, a newly minted programmer hadn't checked in his code for a major testing cycle and I'm sure his manager had a little talk with him. In the wee hours of the next morning, he checked in the late code and sent an apology to the whole division for the delay. His email's closing words: "I'm sorry for any incontinence I may have caused." Error? Maybe not.
