I thought I'd add a tidbit to this discussion. We found that you have to
use the "Create named destinations" option any time that you create new
content using copy and paste or Save As. If the cross-reference markers
in one spot get copied to or saved to another location in the same book,
Frame gets confused because the markers are no longer unique.
If for some reason, you can't use the named destinations option, you can
make the problem go away by deleting the cross-reference marker in Frame
at the target location for the hyperlink that doesn't work. When you fix
the resulting unresolved cross-reference, Frame creates a new marker at
that location, eliminating what confused it.
