Problem with TOC in Word

Somebody please help me.

All the Level 2 headings in my TOC are right, except one.

That one appears as:

<TAB> Heading title ...................#

I'm on a deadline! Do you know what could be wrong? I've done everything
you can imagine including completely deleting the text of the heading,
clearing the formatting, retyping it, everything.
