Are you looking for a contract for work, or are you looking to have a 
contract that you can use when handling work that can make the optimum 
use of the 20 questions that are involved with the 1099 form.

You might see if "NOLO Press" in Oregon might have such a form.  They 
have a llot of forms and this might be one.

Otherwise, you might take the 20 Questions and make them part of a 
contract/agreement youwould discuss with your potential client.

Others may have additional information for you.

There is also a magazine (might be out of print) that is called 1099.  
It might have some information you can use.
