Word and Excel Insertion

I have to create a data in-and-out Word template.

In the Word template there has to be data-entry fields
mixed with text descriptors what to enter in each
field.

[ ] enter square feet of damaged carpet
[ ] enter square feet of damaged sheetrock
[ ] enter square feet of damaged subflooring

In the Word template there has to be an embedded
spreadsheet
that takes these data fields and crunches the numbers,
based
on unit pricing embedded and regularly updated back at
HQ.

[ ] = GET square feet of damaged carpet x $35/sf -
POLICYDEDUCTIBLE
[ ] = GET square feet of damaged sheetrock x
$18/sf - POLICYDEDUCTIBLE
[ ] = GET square feet of damaged subflooring x
$42/sf - POLICYDEDUCTIBLE

In the Word template there has to be data-output
fields.

[ ] insured value of carpet loss
[ ] insured value of sheetrock loss
[ ] insured value of subfloor loss

They don't want to use just a spreadsheet or a
data-base entry
because they can't format the printout into a claim
document
that's also readable in MSOffice.

And they don't want to pay for data re-entry into a
Word doc
because of the predictable error rates hitting their
quotes.

And they have to have everything in one document file,

not a Word linked to a remote Excel spreadsheet(s)
where the link might get broken.

I just need to know if it's possible, and maybe a hint.
