Create a document that contains your title page, copyright page, and any 
other front matter, including a TOC field.  Follow all that with a series 
of RD fields, each of which refers to one of your chapter docs.  Follow 
those with an index field.

It should look something like this:

  My Great Document

  --------section break--------

  Copyright blah blah blah

  --------section break--------

  Contents

  {TOC \h \o  \* MERGEFORMAT }

  --------section break--------

  { RD "chapter01.doc" }

  --------section break--------

  { RD "chapter02.doc" }

  --------section break--------

     ...

  --------section break--------

  { RD "chapter19.doc" }

  --------section break--------

  {INDEX \c 2 \h "A"  \* MERGEFORMAT }

Now just select all (Ctrl+A) and update fields (F9).

Sure, you may have to experiment to get the results you want with headers, 
footers, and page numbers throughout, but it's really not that hard.
