Well, it usually is. It's short for:
What's next?
Who's next?
May I serve the next person in line?
What did you mean it to say?

Moving off topic here, but in my retail experience (in the brusque
Northeast), it is never a question. It is short for "Next customer! Pay
attention! Keep the line moving!" There is no rising pitch at the end,
which would signify a customer.
