Wow, what an opportunity for some input.
I wrote a document for my last contract that was titled "Conducting Peer
Reviews".
There are three (3) types of Peer Review that should be addtressed:
1) Structured Walkthroughs
2) Technical Reviews, and
3) Formal Inspections.
It doesn't make a difference what the 'tool' is that is used to prepare the
document.
What is important is that the people who are doing the reviews are involved
in the 'process', and that the process is agreed to by all concerned.
One of the SEI-CMM Capability Maturity Model (SW-CMM) Key Process Areas at
Level 3 was "Peer Reviews". Very well documented on what must be done.
The follow-on CMMI included similar documentation in each of the KPAs that
addressed the 'review' process.
The sites I have provided here also deal with 'Peer Reviews'. You can do a
'search' at each of these for 'Reviews'
In addition to my list, NASA has an excellent document on 'Formal
Inspections'
There are several organizations that have 'System Development Life Cycle
(SDLC)" documentation that you can obtain over the Internet. These deal with
the review process.
